Mostly I come up with them because they are things I want to do myself, but many features are suggested by others.
I have been using MT as a forum since 2003 (on MT 3.2) and only recently packaged everything up into a plugin (well, I mostly re-wrote from the ground up).

Thanks again, Merv. The problem happened because I tweaked a setting for the MT Forum demo, which kinda breaks Ajax Comments at little. ;) Neither has a bug, but I have either disable Ajax Comments or update my templates to properly support it when replying to forum topics. Thanks again for the report.

Note that just like blog comments, the reply form is at the bottom of the thread. When you click the "reply" button at the top, it just jumps you down to the bottom of the page where the form is. As such, the existing comments don't disappear (just scroll up), although it may seem that way. Also, like the default MT4 Entry templates, the reply (comment) form is hidden by default, unless you are logged in. I don't like this approach and will likely change that in the MT Forum default templates, doing so will reduce confusion and reduce barriers to participation.
Good idea on the threaded comments suggestion.
There is no "navigation on the right" in the current design, so I am not sure what you are referring to. However, I will say that this forum is indeed integrated into a blog.

Bud, I think this is a redirection issue within MT. After you login, MT thinks that you were on an "entry", and tries to determine what the URL for that entry should be. Because the topic is not an entry, it guessing wrong, and sends you to a non-existent page (which may have appeared like a blank screen). I have aworked around this is a few places, but I need to address the "redirect after login" case. Thanks for the report.
Note that the OpenID login did complete, but the redirection was less than ideal. Your topic above was successfully posted using your OpenID.

The TypeKey email thing is the nature of the way TypeKey accounts get stored in the system. Perhaps there is a better way to handle them to avoid confusion. Hiding the email field is an option, but enabling TK users to add their email address is valuable for site owners. I will need to test for any side effects of that.
Regarding the Extended Profile, what browser are you using? I wonder if I need to tweak the CSS a little for cross-browser support...

I just cleaned up the comment spam on this entry. Seems like I forgot to enable the "Spam Protect" feature of Ajax Comments -- whoops. ;) I have now enabled that feature, which should prevent most spam...
